Our Services
Medical: Full service family practice including pre-natal, family planning, women's health, children's health, geriatric care, immunizations, physicals, chronic conditions management, and specialty care including podiatry, chiropractic and acupuncture. We are equipped to provide acute walk-in care, and when needing hospitalization, patients are followed by the Healdsburg, Santa Rosa or Ukiah hospital physicians. Our physicians attend to any patients that are admitted to the Cloverdale Healthcare Center.
Behavioral Health: Individual, family and group counseling available
Dentistry: Child and adult dental care including exams, x-rays, cleaning and treatment
Community Outreach: We provide free assistance with various program enrollment applications, and guidance, information and assistance with obtaining other public benefits.
Laboratory: An independent lab draw station operated by Hunter Laboratories is located within the Medical Center and is available to all, regardless if they're a client of the Center.
Fees:
We accept a wide range of private insurance and public programs in addition to accepting private pay and offering a sliding scale discount based on income eligibility.
Private Insurance: most accepted, please contact uscontact us or your insurance carrier. Sorry, we do not accept private insurance in our dental department.
Private Pay: reasonable fees, due at the time of service.
Sliding Scale Discount: available to those who qualify based on federal poverty guidelines. Income verification and screening is required as is enrollment in any applicable public programs
Public programs: Medi-Cal / Medicare / Family Pact / CMSP / Healthy Families / Healthy Kids / CHDP and BCCTP
